Where the Phrase Type 3 Diabetes Comes From

You may have seen the claim that Alzheimer’s disease is really diabetes of the brain. The phrase got its start in the mid-2000s, when a team led by Suzanne de la Monte at Brown University reported that brain tissue from Alzheimer’s patients showed reduced insulin signaling and insulin resistance in the hippocampus and cortex. Their papers used the term «type 3 diabetes» as a working hypothesis, not a clinical diagnosis.

That distinction matters. As of now, no medical body recognizes type 3 diabetes as a formal condition. The American Diabetes Association lists type 1, type 2, and gestational diabetes; it does not list a type 3. So when someone asks «is there a type 3 diabetes,» the honest answer is: there is a research concept with that nickname, but it is not a diagnosis you can receive from a clinician or bill to insurance.

The underlying biology is real, though. Insulin receptors are dense in the brain, especially in areas tied to memory. Insulin helps neurons take up glucose and appears to influence the clearance of amyloid-beta. In some Alzheimer’s brains, those signaling pathways look blunted. Whether that is a cause of the disease, a consequence, or a bystander is still unsettled.

What the Research Actually Shows

Correlation, not yet causation

Large population studies do link type 2 diabetes to a higher risk of dementia, with some estimates putting the increased risk at roughly 50 to 60 percent. That association is one reason the type 3 idea gained traction. But type 2 diabetes also raises the risk of vascular disease and stroke, both of which damage the brain independently. Teasing apart how much comes from insulin resistance in the brain itself versus damaged blood vessels is difficult.

Trials of intranasal insulin for mild cognitive impairment have produced mixed results. Some early studies showed modest memory improvements; larger follow-ups have been less encouraging. That is a long way from proving that treating the brain like a diabetic organ reverses Alzheimer’s.

Why the label can mislead

The biggest practical risk is that people hear «type 3 diabetes» and assume a standard diabetes drug or a strict low-carb diet will prevent or cure dementia. No medication is approved for treating Alzheimer’s on the basis of insulin resistance. If you have concerns about memory changes, the workup should include a real cognitive evaluation, blood work for vitamin B12, thyroid function, and standard metabolic markers, plus a review of medications that can cloud thinking.

Getting Your Blood Sugar Checked in the Standard Way

If a family history of either diabetes or dementia has you worried, the useful step is ordinary metabolic screening, not a hunt for brain insulin tests that don’t exist commercially. If you are wondering where you can get a diabetes test, the options are straightforward: a primary care visit with a fasting glucose and A1c, a community health center on a sliding scale, or a retail pharmacy clinic that offers A1c screening for around $30 to $70. An A1c test reflects average glucose over roughly the previous three months and does not require fasting.

Diagnostic thresholds are worth knowing. An A1c of 5.7 to 6.4 percent indicates prediabetes; 6.5 percent or higher on two occasions supports a diabetes diagnosis. Fasting glucose of 100 to 125 mg/dL is impaired fasting glucose, and 126 or above points to diabetes. These are the numbers clinicians act on, and they are the same whether or not someone is worried about their brain.

For people already managing diabetes, tight control in midlife appears more protective for cognition than aggressive control started in the late 70s, when low blood sugar episodes become a bigger hazard. A common mistake is chasing very low A1c targets in an older adult who lives alone; severe hypoglycemia can cause falls and confusion that outweigh the benefit.

What You Can Act On Now

The habits that protect the vascular system also protect the brain, and the evidence for them is stronger than anything specific to the type 3 hypothesis. Blood pressure under control, regular aerobic activity, enough sleep, and treating hearing loss all have better support for dementia risk reduction than any supplement marketed as a brain insulin aid. If you smoke, quitting is the single largest lever.

Be skeptical of products that borrow the phrase. A clinic advertising a «type 3 diabetes protocol» with unlisted IV infusions or proprietary supplements is selling a hypothesis, not a treatment. Legitimate care for memory complaints looks like a neurological exam, lab work, and a plan that addresses blood pressure, glucose, and medications.

One more note for anyone reading up on this topic: the search results mix genuine research with content about weight-loss drugs and hormone therapies. Those are separate subjects. Semaglutide and similar drugs are being studied for Alzheimer’s in large trials, and results are pending, but no one should start or stop a medication based on a nickname for a hypothesis. Ask your clinician what your own A1c, blood pressure, and cognitive screening results mean together, and treat the type 3 label as a question science is still working through, not an answer.
